If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room.

Telehealth is not a substitute for in-person care in all situations.


Arbitration and Class Action Waiver Notice

These Terms include a dispute resolution and arbitration section that affects your rights.

In most cases, disputes must be resolved by individual arbitration and not as a class action.

These Terms

These Terms and Conditions (Terms) govern your access to and use of this website and any pages where these Terms are posted (Website), plus any related online tools, portals, forms, messaging, scheduling, and payment features made available through the Website (Platform).

The Platform and certain administrative support functions may be provided using technology and operational support services from a third-party telehealth operations provider.

By clicking “I Agree,” “Continue,” “Proceed,” “Register,” “Sign Up,” or by otherwise using the Website or Platform, you agree to these Terms and any policies referenced within them.

If you do not agree, do not use the Website or Platform.


Definitions

  • Company, we, us, or our means JD Squared Wellness, Inc.
  • You or your means the individual using the Website or Platform
  • Clinician means an independent licensed healthcare provider
  • Third Parties include clinicians, pharmacies, labs, shipping carriers, payment processors, and vendors
  • Content refers to all materials on the Website or Platform
  • User Content means any information you submit

Website Content and Informational Use

The Website may include general information about services, telehealth, and wellness topics. This information is for general informational purposes only.

We do not provide medical advice through the Website. Always consult a qualified healthcare professional.


What We Do and Do Not Do

We provide technology and administrative services that help users access telehealth-enabled care, including:

  • Account creation
  • Intake collection
  • Scheduling support
  • Customer service
  • Payment processing

We are not a medical provider and do not diagnose, treat, or prescribe.

Clinical services are provided by independent Clinicians who are solely responsible for medical decisions.


Platform Support and Contacts

The Platform may be supported by Healthaide LLC, which assists with technical operations, intake, scheduling, communications, and payment processing.

JD Squared Wellness, Inc. provides general, non-clinical support such as account access and customer service.

Healthaide LLC supports clinical routing but is not the treating provider.

Telehealth Consent

You may be required to accept a separate telehealth consent before receiving services.


Changes to Terms

We may update these Terms at any time. Continued use of the Website means you accept those changes.


Availability and Eligibility

Services may not be available in all locations and may require identity verification.


Account Registration and Security

You agree to provide accurate information and keep your login credentials secure. You are responsible for all activity under your account.


Electronic Communications

You consent to receive communications via email, phone, SMS, or platform messaging.

Message and data rates may apply. You may opt out of SMS by replying STOP.


Not for Emergencies

The Platform is not monitored for emergencies. Call 911 if needed.


Your Responsibilities

You agree to provide accurate, complete health and personal information and to follow clinician instructions.

Failure to do so may result in delays or adverse outcomes.


Telehealth Limitations

Telehealth may not be appropriate in all cases. Clinicians may require additional testing or in-person care.

We do not guarantee prescriptions, treatments, or availability of services.


Prescriptions and Fulfillment

If prescribed, medications may be sent to a pharmacy of your choice.

Pharmacies are independent Third Parties responsible for fulfillment.

Prescription products are generally non-refundable.


Laboratory Services

Lab services may involve independent providers and may produce inconclusive or delayed results.


Products and Shipping

Shipping timelines are estimates only. You are responsible for accurate delivery information and proper handling of products.


Fees and Payments

You agree to pay all fees associated with services, including consultations, products, subscriptions, and applicable taxes.

You authorize charges to your selected payment method.


Subscriptions

Subscriptions automatically renew unless canceled. Canceling stops future billing but does not refund past charges.


Cancellations

You may cancel per the terms shown at checkout. You remain responsible for services already rendered.


Refunds and Disputes

All sales are final unless otherwise required by law.

Non-refundable items typically include:

  • Clinical services rendered
  • Processing fees
  • Prescriptions and fulfilled products

User Content

Dispute Resolution and Arbitration

Disputes must first be attempted to be resolved informally by contacting support.

Most disputes will be resolved through binding individual arbitration in Tennessee.

You waive the right to a jury trial and class actions.

To opt out of arbitration, email admin@curatarx.com within 30 days of acceptance.


Time Limit for Claims

Claims must be filed within 1 year.


Force Majeure

We are not liable for delays caused by events beyond our control.


Assignment

We may transfer these Terms in connection with business changes.


Severability

If any part is invalid, the rest remains in effect.


No Waiver

Failure to enforce a provision does not waive it.


Entire Agreement

These Terms and related policies form the complete agreement.
