This Telehealth Consent and Consent to Treat (Consent) explains how telehealth works and obtains your informed consent to receive healthcare services using telehealth technologies.

You also authorize any person who accepts this Consent using your account or device to act on your behalf.
Emergency Notice
Telehealth is not for emergencies.

Parties and Roles
Curata Rx LLC. provides general and non-clinical support, including account access, navigation, and administrative assistance.
Clinical services are provided by independent licensed healthcare professionals (Providers). Providers are solely responsible for clinical decisions, including treatment and prescriptions.
We work with a clinical support organization that helps facilitate access to Providers and supports care operations, including prescription coordination. Healthaide LLC performs this role.
We are not your treating clinician and do not provide medical care directly.
Definitions
- Telehealth means delivering healthcare using electronic communications when you and your provider are not in the same location
- Service refers to the platform, tools, and systems supporting telehealth
- You refers to the patient receiving services
How Telehealth May Be Used
Telehealth may be used for:
- Evaluation and consultation
- Diagnosis and treatment planning
- Prescribing decisions
- Follow-up care and monitoring
- Patient education
Interactions may include:
- Audio or video communication
- Messaging and intake forms
- Sharing records, images, and lab results
Your Provider may determine telehealth is not appropriate and may direct you to in-person care.
Alternatives to Telehealth
You may choose in-person care instead of telehealth when available.

Your Responsibilities

Protected health information (PHI) is handled by Providers under applicable laws, including HIPAA where applicable.

Providers may not be immediately available. Response times vary.

Location and Eligibility
